What is a free QA plan template?
A free QA plan template is a reusable structure for setting out how quality will be achieved on a piece of work, who is responsible for each part of it, and what evidence will exist that it happened.
The definition contains a distinction most templates lose. Achieved, not verified. A plan describing how you will check the finished work is a different and much weaker document than one describing how you will make the work correct in the first place, and the majority of documents titled QA plan are the former wearing the name of the latter.
The template is not the plan. It is an empty structure, and what determines whether the finished document does anything is not its formatting but the balance between activities that happen before the work and activities that happen after it.

QA plan or QC plan? Sort your activities by when they happen
Take your draft plan and put every activity in it into one of three columns. Before the work. During the work. After the work.
Most plans, sorted this way, are almost entirely in the third column. Final inspection, sample testing, audit, review, sign off, defect log. Each is a real activity and each is quality control, which means finding defects that already exist.
Quality assurance is the first column. It is everything that makes the defect less likely to be created: how the reference documents reach the people doing the work, whether the people are qualified for it, whether the tools are calibrated, whether the requirements were unambiguous, whether the work can be done at the speed being asked of it.
Both are necessary. A plan with no control has no feedback, and a plan with no assurance is a promise to find problems rather than to avoid them.
The test is simple and uncomfortable. Count the activities in each column. If the third column has four times the entries of the first, your plan cannot prevent anything. It can only tell you, eventually, how often you were wrong.
The second column matters more than it is given credit for, because in process checks are the cheapest place to catch something. A defect found during the work costs the work since the last check. A defect found after the work costs all of it. A defect found by the customer costs all of it plus the relationship.

Which QA plan do you need?
QA means two quite different things and the search results for this term mix them freely, so it is worth deciding which document you are actually writing before you adopt a template.
Quality assurance in the general sense. How a project, a product or a service will meet its requirements. Covers process, standards, roles, records and audits. This is what manufacturing, construction, consulting and most regulated industries mean by a QA plan, and it is what this page is primarily about.
Software QA. How software will be tested. Test scope, environments, test types, entry and exit criteria, defect handling. Frequently called a QA plan and closer in kind to a test plan. Covered in its own section further down, since the two are commonly confused with each other as well as with this document.
Quality Assurance Project Plan. Searches for a Quality Assurance Plan Template free download frequently land here without meaning to. This is a formally specified document in environmental and scientific research, particularly under United States Environmental Protection Agency requirements, with mandated content and structure. If this is what you need, the specification governs and no general template on any website will satisfy it. Work from the issuing body's own guidance.

What a QA plan template must contain
Nine components. The activity table carries most of the weight.

Reference control is the row that most plans omit and the one that caused the failure described below. Every inspection compares something against a reference. If the reference itself is not controlled, the inspection is confirming conformance to the wrong thing, thoroughly and with documentation.

Three before, two during, two after. That balance is the point of the table, and it is deliberately the reverse of what most plans contain.
Acceptance criteria. All dimensions within drawing tolerance. Surface finish to drawing note 4. Any part outside tolerance rejects the sample and triggers full inspection of the batch.
Nonconformance handling. Operator stops and tags the part. Inspector raises a nonconformance record within one hour. Production on that job halts until disposition. Only the Quality Manager decides use as is, rework, or scrap, and use as is requires written customer agreement.
Records. First article reports, check sheets, inspection reports and certificates retained seven years per contract clause 9, held in the quality system, retrievable within one working day.
Review. Reviewed at contract milestones, and immediately on any customer complaint, any drawing revision, or any change of machine or material.

QA plan example: four thousand parts and every inspection passed
Nethergate Precision, a contract machining business of about a hundred and eighty people, won an automotive contract and wrote a quality plan for it.
The plan ran to twenty two pages. It specified first article inspection, in process checks at three stages, final inspection to an agreed sampling standard, gauge calibration, operator qualification and full records. It passed the customer's supplier audit without a single finding.
Eight months later they shipped four thousand housings machined to drawing revision E. The customer had issued revision F eleven weeks earlier.
Every inspection had passed. First article, in process, final, all of them, because each one compared the parts against the drawing in the inspector's folder, and the folder contained revision E. The parts were made correctly. They conformed precisely to a document that had been superseded.
The cost was a hundred and forty five thousand pounds across recall, rework, expedited freight and the customer's own line stoppage, and the account moved to enhanced supplier monitoring for a year.
The review found the thing that makes this example worth reading. Of twenty three quality activities in the plan, nineteen happened after a part had been cut. Four happened during. Not one happened before, and nothing anywhere in twenty two pages described how a drawing revision travelled from the customer to the person setting the machine.
The plan was thorough about verifying conformance and silent about assuring the reference. So a change in the reference passed through every control undetected, because none of the controls were pointed at it.
What they changed was not more inspection. They added four activities to the front of the plan. Drawing release through one route only. Revision letter printed on the job card and cross checked at setup by a second person. Superseded drawings destroyed rather than filed, since a filed obsolete drawing is an obsolete drawing waiting to be picked up. And a weekly reconciliation between the customer portal and the floor register.
Two consequences followed, and the second one surprised them.
There were no further reference escapes in the eighteen months after. And total inspection time went down, because one of the in process checks had existed solely to catch this class of problem late and was removed as redundant once it was being caught early.
Assurance is usually cheaper than the control it replaces. That is the argument to make to whoever thinks quality is a cost.
How to write a QA plan in six steps
Name what the work is measured against. Specifications, drawings, regulations, acceptance criteria, by version. If this cannot be stated precisely, nothing downstream will be precise either.
Write the reference control before anything else. How those documents reach the people doing the work and how old versions are removed. This is the step almost everyone skips.
List every quality activity you intend. Do not organise them yet. Just list them.
Sort them into before, during and after. Look at the shape. If the third column dominates, go back to step three and ask what would make each defect less likely rather than more findable.
Attach evidence to every activity. An activity producing no record cannot be shown to have happened, which in an audited environment means it did not.
Name who can reject. A plan where nobody has the authority to stop work is a description rather than a control.
Step four is the whole exercise. Everything else is bookkeeping around it.
Where defects get caught and what each stage costs
The argument for assurance over inspection is economic rather than moral, and it is worth putting numbers on for whoever approves the budget.
A defect prevented costs the price of the control that prevented it.
A defect caught during the work costs the work done since the last check. On a batch process with a check every fifty parts, that is fifty parts.
A defect caught at final inspection costs the whole batch, plus the schedule.
A defect caught by the customer costs the batch, the freight both ways, the replacement, whatever their disruption cost them, and a change in how they think about you. This is the only stage where the cost is not bounded by anything you control.

QA plan template variants: software, construction, manufacturing and regulated
The structure holds and the emphasis moves substantially.
Manufacturing quality plan. Reference control, gauge calibration, operator qualification and sampling. The example above is this variant.
Construction quality plan. Inspection and test plans, hold points, witness points, and third party inspection. Frequently contractually required in a specified format, and the format overrides anything on this page. Software QA plan. Test scope, environments, entry and exit criteria, defect severity and triage. Assurance here means code review, static analysis, definition of done and requirements clarity, all of which happen before a test is run. Service and call centre QA plan. Scorecards, call sampling and coaching. Assurance is script quality, training and system design. Control is the scorecard, and organisations in this sector tend to have far more of the second than the first.
ISO 9001 quality plan. Adds document control, internal audit, management review and corrective action obligations. Document control is the reference control row made mandatory, for exactly the reason the example above demonstrates.
Regulated quality plans. Medical devices, pharmaceuticals, aerospace, food and environmental research all have mandated content, and in several cases mandated structure. Nothing on this page is a substitute for the applicable standard or for review by a qualified quality professional. Where a regulator or a customer specifies a format, theirs governs.
QA plan, test plan or test strategy?
Three documents that overlap heavily in software and are worth separating.
A test strategy is organisational and long lived. How this company tests, what types of testing it uses, what its standards are. Written once, revised rarely, and applying across projects.
A test plan is project specific. What will be tested on this release, in which environments, by whom, against what entry and exit criteria, with what schedule.
A QA plan is broader than both when used properly, because it covers the assurance activities that reduce defects before testing begins. Requirements review, definition of done, code review standards, environment parity.
In practice many organisations use QA plan and test plan interchangeably, and the result is that the assurance half never gets written, because a document named for testing naturally fills up with testing. If you are producing a QA plan for software, apply the sorting exercise above and check how much of it happens before the first test case runs.
Who owns the QA plan and when to review it
One named person owns it, and it should be someone who can stop work. A plan owned by somebody with no authority to reject is a document rather than a control.
It is not a living document in the way a schedule is. Review it at defined points rather than continuously, and revise it when something it depends on changes: a new revision of the standard, a change of process, a new machine or environment, a customer complaint, or any nonconformance that the plan did not catch.
That last trigger is the useful one. Every escape is evidence about the plan, and the question to ask afterwards is not who missed it but which column the missing control belonged in. The answer is almost always the first.
Show your quality method rather than describing it
Quality plans describe controls. What they cannot convey is how a control is actually performed, and a control performed differently by three people is three controls.
This is the gap that audits find and that written procedures do not close, because the reader has to reconstruct a physical or on screen sequence from prose, gives up, and copies whoever is nearest.

What a free QA plan template cannot fix
A plan written for an audit rather than for the work. These are recognisable by being complete, well formatted and unconnected to what anyone does. They pass audits and prevent nothing.
No authority to stop. If the person who spots a problem cannot halt the work, the plan describes a reporting process rather than a control.
A template chosen on appearance. The best free QA plan template is rarely the best looking one, since the two sections that matter, reference control and authority to stop, are plain text and add nothing visually.
Unmeasurable acceptance criteria. "High quality finish" cannot be assured or controlled. Somebody will decide it on the day, differently each time.
